Moriarty finally made its presence known in Watson Episode 7, and he is after John once again. The previous episodes saw him poaching the doctor's fellow Shinwell and asking him to replace his medicines.

While Shinwell decides not to betray John anymore, Moriarty blackmails him and sends him a photo of his parents back in England, hinting that he will harm him if Shinwell does not comply with him. He finally gives in and replaces John's medicines, which causes him extreme auditory hallucinations.

While working on the case of the week, John hallucinates about the late Sherlock Holmes and hears his voice echoing all around him. While he figures out that somebody has messed with his medicines, a pharmacist named Lloyd takes the blame for Shinwell, presumably on Moriarty's orders.

Shinwell fears for his safety and the fact that John might catch up to him, but before the two can confront Lloyd, he reveals that he has a drug overdose and died on their clinic's floor.

Sherlock Holmes' auditory hallucinations guide Watson in the case of the week

Watson Episode 7 saw a bizarre patient, Ginny, who was suffering from short-term memory loss, where her memory would reset every three minutes, making it impossible for her to explain her problem to the ER doctors.

John decides to treat her and gets hold of her planner, which might reveal key details about her life. He finally catches onto a particular detail, where Ginny complains that something is "eating her from the inside."

John again experiences an auditory hallucination about Sherlock, where the detective tells him something about teeth marks and how they can have lasting impressions. This helps John to take the case in the right direction, and an X-ray reveals that Ginny had ovarian teratoma, a tumor with teeth, which is why she was feeling that something was eating her from the inside.

John immediately orders Ginny's surgery, and her tumor is removed, after which her memory is restored to normal.

Moriarty targets Ingrid in Watson Episode 7

Moriarty finally made a physical appearance in Watson Episode 7. After realizing that Shinwell is becoming too risky to target John from the inside, he seems to have set his sights on Ingrid, John's skilled neurologist.

Throughout Episode 7, Ingrid receives mysterious text messages about a pond. The messages reveal that the person knows she spends her birthday by the pond. After finishing her shift, she heads for the pond, only to find Moriarty waiting there for her.

He reveals that he knows all about her secretive past and the fact that the body buried deep under the pond has a connection with her, hinting that Ingrid might have murdered and buried the body there.

He also hands the body's corroded finger wrapped in a plastic bag, hinting that he will now use this as leverage and force Ingrid to work for him and target John.
