Morris Chestnut stars in the series as Dr. John Watson, a physician and former detective who runs a medical clinic in Pittsburgh. He applies the deductive thinking he acquired from Sherlock Holmes to cure patients with unusual diseases. Eve Harlow portrays Ingrid Derian, a neurologist with a murky past and a strained relationship with her coworkers. Peter Mark Kendall plays identical twins Stephens and Adam Croft, infectious disease doctors with distinct personalities.

Sasha Lubbock, played by Inga Schlingmann, is an immunologist and rheumatologist who battles with her identity between her biological and adoptive families. Shinwell Johnson, portrayed by Ritchie Coster, is a former criminal who lends the clinic his useful connections. Mary Morstan, Watson's ex-wife and the medical director of the clinic, is portrayed by Rochelle Aytes. Despite their divorce, they still collaborate closely.

Randall Park plays Moriarty, Sherlock Holmes' iconic adversary who returns with a terrible agenda. Matt Berry portrays Sherlock Holmes, who appears in hallucination scenes. The show combines medical drama and detective aspects, centering on Watson's quest to discover the truth about Holmes' death and Moriarty's reappearance.
