In the wake of Juliette's exit from the Silo and her venture into the open world outside, things were brewing with tension within the vault. Bernard tried his best to maintain the status quo and constrain dissent. To be true to the Pact and the Order, he ended up murdering Judge Meadows.
Although Judge Meadows was presented as the head of The Judicial on Silo season 1, it was soon realized that she doesn't have much real power to wield. Thereafter the first season of the Apple TV+ show kept her on the sidelines.
In the second season, she began assuming a greater role after she gave up on her supply of alcohol and decided to be sober. Seeds of dissent soon arose within her mind as her previous notions of the vault being the only inhabitable place were shattered following Juliette's escape.
Judge Meadows soon grew irreconcilably drawn to the outer world and kept asking Bernard for the suit that will allow her to survive outside the Silo. Here's all that we know about her death.
How did Judge Meadows meet her fate on Silo season 2?
Juliette's escape from the vault on Silo season 2 served both as the inciting incident and a watershed moment for the other dwellers of the vault. All other previous instances of inmates exiting the vault were followed by the act of cleaning the lens, which maintain the illusion for the inmates in the vault that the world outside was unforgivable and inhabitable. Juliette became the first person to venture farthest from the vault and yet live to tell the tale.
Judge Meadows soon began pining for the outside world and set about in her attempt to turn her life around. All these years she had served as Bernard's shadow and when the Mayor asked for her to help in his efforts to restore decorum inside the vault, she agreed on the condition that he would give her the suit that would allow her to survive outside the vault. She also met with the people from the Mechanical level and this was a grave transgression on her part.
Bernard was eventually left with the option of either agreeing to free Meadows or to put a permanent end to her. Regardless of his relationship with her, he chose the latter option. He lured Meadows into his room on the pretext of giving her the suit.
He had prepared a meal of mushrooms for her which was laced with poison. Meadows began feeling the effects of the poison soon after taking a bite and in her final moments, Bernard tried to comfort her with a virtual reality headset that flashed images of a beach in Costa Rica from 2018.

Mayor Bernard and Judge Meadows have been working together for a long time inside the Silo. Their personal and professional lives have been intertwined. The scene in which Bernard sensually takes her measurements for the suit goes on to prove that they might have feelings for each other. However, in his pursuit of honoring the Pact and the Order, Bernard contended himself with having Meadows as his shadow.
