Apple TV's Severance Season 2 confirmed a dark theory about Helly R., which Reddit fans have speculated since the Season 2 premiere.
In Episode 4, it was revealed that Helena replaced her innie, Helly R, with her outie and has been spying on her colleagues through this.
But this revelation cost Irving's innie's life because after he proved that Helly R. is actually Helena Eagan, Milchik fires him from Lumon, thus ending his innie's journey on the show.
While Irving's fate in Episode 4 was a shocking twist for many fans, it was already subtly foreshadowed in Episode 2. In a scene when Milchik is shown beheading Irving with his motorcycle, the makers might be foreshadowing Irving's fate in the upcoming episodes.

Irving's fate was already foreshadowed in Severance Season 2
After the shocking finale of Severance Season 1, it was shown that Dylan and Irving's innies were fired from Lumon, but the Season 2 premiere saw Mark asking to bring back his other colleagues.
While Irving is brought back to Lumon, there is a subtle foreshadowing that his journey at Lumon will be cut short, which ultimately happens in Episode 4. Back in Episode 2, there was a small transitional scene where Milchik can be seen riding his motorcycle, which goes all the way through Irving's throat, making it look like he is beheading or 'severing' him.
While the scene appeared weird to many viewers at that time, many fans have now linked it with Irving's fate in Episode 4. Milchik beheading Irving in Episode 2 was a foreshadowing of how he will fire/kill his innie in the upcoming episode.
Irving's fate in Severance Season 2 Episode 4 suggests that his innie is dead
Milchik not only fired Irving in Severance's latest episode but essentially killed his innie and removed his trace from Lumon's history forever. As shown in the show's universe, once a severed Lumon employee is fired, his innie gets killed and he is wiped from the memories of his fellow employees.
After Irving threatens to kill Helena/Helly, Milchik terminates him in the final moments of Episode 4:
“You have threatened collegial murder in the pond of Woe’s Hollow...For this, there can be no penalty but immediate and permanent dismissal. It will be as if you, Irving B., never even existed nor drew a single breath upon this Earth.”
Milchik's statement makes it clear that Irving's innie is not only fired but he is killed by Milchik because it will be as if he never existed in Lumon's history.
While we may not see Irving's innie in Severance Season 2 anymore, his outie might take the spotlight now, as it was hinted that he knows about Lumon, especially the Exports Hall.
While it is still unclear how Irving's outie knows about Lumon, it will be interesting to see Irving's outie's next steps after his innie is killed in the latest episode.
