In the episode, the doctors deal with a new sensitive case — the separation surgery of Elijah and Powell. A fluid begins to ooze inside Elijah's chest, and the doctors discover there is a tumor growing fast inside him and could potentially spread to his heart.
Chief of emergency medicine, Dr. Caitlin Lenox (played by Sarah Ramos), suggests shifting the surgery, but Elijah decides that he doesn't want Powell to suffer because of his illness, and the brothers agree to get the surgery done.
It was a heart-wrenching moment of irony as Elijah survives the surgery while Powell ends up suffering a stroke and is put on life support.
